Abstract
Laparoscopic management of the common bile duct stone had been performed successfully in 29 patients(27 choledocholithiasis and 2 Hepatolithiasis) by 28 LCBDE and 1 laparoscopic transcystic duct stone removal from March 1991 to June 1992.
Theoretically, a simultaneous Laparoscopic cholecystectomy(LC) and LCBDE is very attractive because the disease can be cured in one session on the operating table. According to our 29 experiences LCBDE shares the same basic sugical principles
with
the
conventional open choledochotomy, but is technically much demanding.
Because the ability to perform LCBDE allows the surgeon not to let the patients being undergone an open surgery when unsuspected CBD stones are found intraoperatively, the surgeon who is performing LC has to be familiar with this procedure.
However, when CBD stones are found preoperatively, a preoperative ERCP sphincterotomy and subsequent laparoscopic cholecystectomy seems to be more advantageous than a simultaneous laparoscopic cholecystectomy and LCBDE from the patients's point
of
view.
